Hazard, Hazard Identification and Risk Analysis

Hazard:

A characteristic of the system/plant process/substance that represents a potential for an accident causing damage to people, property or the environment.

Types of Hazards:

Hazardous Substances:

Consequences/Outcomes of hazards:

FireExplosionOthers
Jet FirePool FireFlash FireVapour Cloud ExplosionBoiling Liquid Expanding Vapour ExplosionToxic substance ExposureOccupational Diseases

Consequence: Adverse effects or harm which causes the quality of human health or the scale environment to be impaired.

Hazard Assessment:

The process whereby the results of an analysis of a hazard are considered against either judgment, standards or criteria developed as a basis for decision making.

Hazard Control Measures

  • Remove the hazard
  •  Substitute
  •  Isolate
  • Add engineering controls
  •  Safe work practices
  •  Provide PPE
  •  Implement and monitor controls

Risk:

A measure of potential economic loss or human injury in terms of the probability of the loss or injury occurring and the magnitude of the loss or injury if it occurs.

Risk Assessment:

The risk determination process is usually in a quantitative or, it is an evaluation of the likelihood of undesired events and the likelihood of harm or damage being caused together with the value of judgements made concerning the significance of the results.

Risk Classification:

A rating used to derive an appreciation of the relative risk from a hazard. The risk matrix may be used to assist to determine this rating, both the probability and the potential consequences are categorised by 5 point scales. The product of the two is the risk classification as given in the following Matrix.

Risk Tolerance Criteria

Risk Management:

The process is described in the following diagram:
